PLEASE READ: IMPORTANT BASICS ABOUT FILLING OUT FORMS

When filling out the forms:

 

IF YOU PRINT THE FORMS OUT AND THEY ARE NOT ON ONE PAGE PER FORM DO NOT SUBMIT. Sometimes a MAC product will do this, you can use Microsoft Word to correct it. Sometimes if you use Google Docs it will mess up the alignment of the form as well. YOU CAN GET A HARD COPY TO FILL OUT AT DRILL FROM ADMIN, be aware this could delay dates for trainings and enrollment. 

 

For example, if you print out pg 3 of the Med History and the bottom of the page is on a second sheet, that is unacceptable.

 

  • Type or use BLACK INK ONLY, on everything if filling it out by hand.

 

  • Dates MUST be entered as DD MMM YY (for example, 18 JUN 19). This means two digit day three letter month and two digit year. If you put 06-05-19 we will not know if you mean June 5, 2019, or May 6, 2019. This is VERY important!

 

  • Cadets USNSCC Unit: Sacramento Division Region: 125SAC

 

  • League Cadets USNLCC Unit: TS California Region: 125CAL

 

  • NO SOCIAL SECURITY NUMBERS ON ANY FORMS. Each cadet or league cadet are issued an individual ID number once they are registered. 

 

Correctly filled out forms will help ensure that your child is promptly enrolled in the program and receives credit for all of their future efforts.

 

The entire application must be returned:

 

  • Every application requires a copy front and back of your medical insurance card

  • Every application requires a copy of immunizations. 

CODE OF CONDUCT

* THIS MUST BE TURNED IN WITH YOUR APPLICATION

*A copy of this form is required to go in your cadet's file it requires a cadet or leaguer signature.

 

* This is a contract that will help define your expectations about what is expected from you in the program. 

NEW CADET/LEAGUE CADET APPLICATION PACKET

If a league cadet HAS FLEETED UP you will NEED a WHOLE new application, that is all the pages. 

The packet includes Member Information and Declarations, Report of Medical History, Medical History Supplemental, Report of Medical Exam, Request for Accommodation, and Parental Support Agreement. (Complete Packet = 11 Pages)

Cadet Service Record

Every Adult and Cadet in the USNSCC & USNLCC are required to have a service record.

The service jacket can be called:

  • service record

  • service jacket 

  • folder

The service record is kept in admin. We are required to have all the documents for the service record per our headquarters administrative manual.

We are inspected annually for the service record to make sure all files are up to date. 

Every training you go to you will need to take this service record with you, if you do not take it with you they have the right to send you home. You will be informed at the drill prior to your training about how to attain your service record. You will also be emailed about completing the required documents prior to training, if your service record is incomplete you will not be able to attend training.

PAGES 1 & 2 - MEMBER INFORMATION AND DECLARATIONS

* Cadet & League Cadet Application (NSCADM 001 pg 1 & 2): 

 

Please leave USNSCC/USNLCC ID box empty we will enter this number when they are registered. 

​* EMAIL: We do 99% of our communication via email. Make sure your email on the application is legible and valid.

  

​* Have your prospective cadet sign in black ink, in section 2a.

​* If you fleet up as a league cadet you will need to fill this out again. Your USNSCC is on your ID card. 

PAGES 3 & 4 - REPORT OF MEDICAL HISTORY

To be completed by a parent or guardian and submitted with the application. 

 

* Please make sure to include all information on this page!

* Leave USNSCC/USNLCC ID box empty we will enter this number when they are registered. You do not need a Dr's signature, only the parent or guardian.

* Initials must be handwritten in BLACK INK ONLY not typed.

*  When you register for a training, Admin will send you this form PREDATED. This is the release of liability for your cadet to be treated medically while at the training. If something needs to be changed within 30 days of the start date of the training we can submit a new form. A wet signature is required upon check-in to the training some medical facilities will not accept a copy. We need to verify the form is correct before sending the original with your cadet to training. COTC's have been authorized to send cadets home if this form is filled out incorrectly or a copy is placed in the file. Help us help you get your cadet off to a great start to trainings!  

 

UPDATED WITHIN 30 DAYS OF THE START DATE OF THE TRAINING.

EVERY TRAINING EVERY TIME. 

PAGES 5 & 6 ONLY - REPORT OF MEDICAL EXAMINATION

* Must be filled out and signed by a Dr.

 

* You MAY take up to 3 months to get the Medical Examination completed, but the sooner it is returned, the sooner your child can fully participate in the program.

 

* Take your child in for a sports physical and have the Dr sign.

 

* If your child has had a physical within the last year you can just have the doctor fill out the form without having to do another physical. 

 

* Please leave USNSCC/USNLCC ID box empty we will enter this number when they are registered. 

 

UPDATED ONCE A YEAR

PAGES 7 & 8 - REPORT OF MEDICAL HISTORY SUPPLEMENTAL

This form is only for medications.

 

* If your child has a prescription this must be signed by the Dr.

* If they take over the counter medication it the parent or guardian must sign it. 

 

UPDATED WITHIN 30 DAYS OF THE START DATE OF THE TRAINING.

EVERY TRAINING EVERY TIME. 

PAGES 7 & 8 MED SUPPLEMENTAL NOT APPLICABLE

* If your child does not take any medication, then you need to draw a single diagonal line from the bottom left corner to the top right corner.

* Write N/A on that diagonal line.

* Regardless of how you fill out this form, it needs a cadet or leaguer name and parent signature.

PAGES 9 & 10 ONLY - REQUEST FOR ACCOMMODATION

* Complete this form ONLY when an accommodation is requested for a prospective cadet under the Americans with Disabilities Act 1 and the disability is PERMANENT, not temporary.

 

* This form is for your child if they do not meet minimum standards due to physical or mental delays. 

PAGES 9 & 10 ACCOMMODATION NOT APPLICABLE

* If your child does not need an accommodation, then you need to draw a single diagonal line from the bottom left corner to the top right corner and write N/A on that diagonal line.

 

* Regardless of how you fill out this form, it needs a cadet or leaguer name and parent signature.  

 

* Below is an example of what it should look like. Use the form above.

PAGE 11 - PARENTAL SUPPORT AGREEMENT

* A copy of this form is needed to go in your cadet's file.

 

*This is not a guarantee that you will be called upon to volunteer.

 

*If you do not want to volunteer you must still fill out the form.

COMMUNITY SERVICE

* This is the form you would use for ALL community service outside the unit.

* If you received an award with a different organization for the hours you are submitting to us they are not valid. 

ACADEMIC ACHIEVEMENT Public School

* According to the ribbons & appurtenances manual in order to receive the ribbon the following letter must be filled out by the school attending. 

* A copy of your report card is required with this letter.

* You can only earn this award every 6 months.

ACADEMIC ACHIEVEMENT Home School

* According to the ribbons & appurtenances manual in order to receive the ribbon the following letter must be filled out by the home school program. 

 

* You can only earn this award every 6 months.

SWIM QUALIFICATION

The criteria on the form meet U.S. Navy basic swim qualification standards.

 

*Must be dated within the year of the training requiring it.

*It is not the responsibility of the unit to get you to swim qualified. You must find a registered swim instructor to fill out this form. 

*It is a great idea to do it even if you are not sure, it really sucks to scramble to get it done in time to register. If a training requires swim qualification you can't register until it is completed.
